a. Location Plan or Sketch of the location clearly showing where business is located.
b. Fire Safety Inspection Certificate for occupancy (BFP)
c. For applicants with valid FSIC for occupancy, Affidavit of undertaking that there had been no substantial changes made on the building/establishment given the FSIC (Requirement of BFP)
d. Clearances, Licenses from National Government Agencies
e. CCTV Installation Compliance Certificate (City Ordinance No.2014-343) (To be secured at City PNP)
f. Office of the Building Official: OBO Clearance
g. City Planning and Dev't Office Zoning/Location Clearance
h. Community Tax Certificate (CTC)
i. Barangay Business Clearance
j. ENRO: Environmental & Solid Waste Management Clearance
k. City Health Office: Sanitary Permit
l. CTO-Land Tax Division: Real Property Tax Clearance
m. Public Employment Service Office (PESO) Clearance
n. Business Plate
o. Clearances from other agencies: (Choose 1 of the ff. If you have employee comply all of the following clearances)

Steps in Application for New and Renewal of Business Permits

1. Register to the website (only for unregistered member only), for registered member proceed to step 2.

a. Upload clear photo of your 1 valid primary ID and 1 secondary ID. Also provide your personal email.
b. Verify your email
c. Pay the 100 Pesos registration fee. (Your 100 pesos that you’re going to pay will be your partial payment of your Business Plate number. ).
d. Fill up the account registration form and click register to submit.

2. Login to your account
3. Select Business permit tab for apply new and renewal of business permit.

a. Select new application for new business permit.
b. Select for renew application for renewal of business permit.

4. Fill up the application and upload the required documents.
5. Submit application form.
6. Check your status of application.
7. If status of application is for payment then pay the full amount using a PayPal account.
8. Wait for a notification on your email account.
9. Once status of application is for release, go to the City Hall business permit licensing office to claim personally your business permit.
